Facilities Commission, that Quality Hospice, a/an Hospice Agency owned by All Care Plus, Inc. with an ownership type of Corporation (For Profit) and to be managed by itself intends to file an application for a
Certificate of Need for the
addition of Anderson,
Campbell, Cumberland,
Putnam, and Roane Counties to its service area (License #621). The address of the project will be 341 West Central Avenue, Suite B, Jamestown, Fentress, Tennessee, 38556. The
estimated project cost will be $46,000.
